I am not sure what happens. I can assume that the motor does not get the oil flow it needs. I hope this is just a what if question. I just know if it says this way do it this way.

Welcome to the forums. If you put the oil filter in backwards, it will block oil flow to the top of your engine and do serious damage. Sometimes it will do the damage quick and other times it might take a little while, but it will definitely damage the engine.
