So, I’ve found a set of Polaris take offs for $150. The rears are 12”, and mine are 10”. Which factory Honda 12” rims will I need, provided I can find some used?
Any Foreman SRA rear rims will work. Also any 2001 to 2014 Rubicon rear rims will work. Your stock front rims will work as-is with those tires you just picked up.

Another option, ITP Delta Steels can be found for cheap online, and look close to stock. That's what I did on the rears of my Rancher.

Another option, ITP Delta Steels can be found for cheap online, and look close to stock. That's what I did on the rears of my Rancher.
Good idea; the Delta rims are decent looking too. Are there any advantages to the 12” rears? Only advantage I see with the 10” is a slightly better ride because of the taller sidewall height.
